Preconception Health Checklist


Read through this list and keep a score. The closer your score is to 10, the more prepared you will be for a healthy pregnancy. If you have to say "I don't know" to a question, give yourself a zero.


1. Have you made a plan about how many children you would like to have, and when you would like to have them?

2. Do you have high blood pressure or hypertension?

Yes 0 No 1

If you are actively controlling your hypertension through medication, diet and exercise, give yourself a 1.

3. Have you been vaccinated against rubella, varicela (chickenpox) and hepatitis B?

Yes 1 No 0

4. Do you take the recommended amount of folic acid (400 micrograms ) every day?

Yes 1 No 0

Folic acid can be found in green leafy vegetables like spinach and collards, orange juice and fortified cereals and bread.

5. Do you smoke cigarettes?

Yes 0 No 1

6. Do you have high blood sugar or diabetes?

Yes 0 No 1

If you are actively controlling your diabetes through medication, diet and exercise, give yourself a 1.

7. Have you been tested for STIs such as Syphilis, Gonorrhea, Chlamydia and HIV in the last year?

Yes 1 No 0

8. Have you seen a dentist in the last year?

Yes 1 No 0

9. If you don't want to be pregnant, are you taking steps to prevent pregnancy such as condoms, birth control, abstinence, etc.?

Yes 1 No 0

10. Could you say with certainty that you know if you are pregnant or not at this moment?

Yes 1 No 0

50% of all pregnancies in America are unplanned. Unplanned pregnancies have a higher risk of poor birth outcomes. Get healthy before you get pregnant - and be pregnant only when you want to be!
